Requesting an Equipment Upgrade or New Device
Purpose
This article outlines the process for requesting an equipment upgrade or a new device to support your job responsibilities.
When to Submit a Request
Submit a request if:
- Your current device is no longer meeting performance requirements
- Your device is damaged or malfunctioning
- You are starting a new role that requires different equipment
- You need specialized hardware to perform job duties
Required Information
Before submitting your request, gather the following details:
- Full Name
- Position/Job Title
- Department
- Manager’s Name

Device Type Requested (e.g., laptop, desktop, monitor, accessories)
Current Device Information (if applicable)
- Asset tag number
- Model
- Age of device
- Reason for Request / Business Justification
- Manager Approval (must be documented)
Submission Process
- Obtain manager approval.
- Compile all required information listed above.

Review & Approval
- IT will evaluate the request based on:
- Device lifecycle standards
- Business need
- Budget considerations
- Additional information may be requested.
- Once approved, IT will coordinate procurement and setup.

- Standard device replacements typically follow company lifecycle guidelines.
- Specialized or non-standard equipment may require additional approval.
- Equipment must be returned to IT if being replaced.
