Requesting Equipment for Clinic


Purpose

This SOP outlines the standardized process for requesting equipment for clinic staff to ensure timely fulfillment and proper tracking.


Submission Methods

All equipment requests must be submitted through one of the following methods:

  • Submit a ticket via: help.behavioralframework.com
  • Email: itsupport@behavioralframework.com

Required Information

When submitting a request, include the following details:

  • Name of person receiving equipment
  • Title/Role of the recipient
  • Requester’s name
  • Description of equipment needed

Incomplete requests may result in delays.


Special Instructions for North Carolina (NC) Clinics

If the request is for an NC clinic:

  1. Update the "Technology Needs in NC" document

    Clearly indicate:

    • The employee’s name
    • The clinic location
  2. Highlight the entry for visibility
  3. A ticket must still be submitted (this step does not replace the ticket requirement)

Process Overview

  1. Gather all required information
  2. Submit a ticket or send an email request
  3. (NC Only) Update and highlight the Technology Needs in NC document
  4. IT reviews and processes the request

Key Notes

  • All requests must go through IT for tracking and approval
  • Do not bypass the ticketing system, even for urgent requests
  • NC clinic requests require both documentation update and ticket submission
